Who We Are
CoreHealth Partners is a specialist consultancy dedicated to strengthening NHS Primary Care, Community Services and Adult Social Care – underpinning the Neighbourhood agenda.
We exist to close the gap between national policy ambition and frontline reality.
Our team brings together deep, real-world expertise in digital transformation, clinical safety, strategic analytics, governance, quality and compliance, workforce leadership, and integrated care delivery. We work at the point where operational pressure, regulatory expectation, and system reform collide – supporting organisations to stabilise performance, build maturity, and create sustainable, neighbourhood-based models of care that genuinely improve outcomes.
Our Mission:
To strengthen neighbourhood care systems, empower frontline teams, and build the foundations for safer services, better outcomes and sustainable care for communities.
